Gout, also known as hyperuricemia, is a chronic metabolic disorder that results from excessive levels of uric acid in the blood plasma. It is often accompanied by obesity, hypertension, and type 2 diabetes. Gout is typically asymptomatic for most of the time, but after a certain period, an attack occurs - intense pain and inflammation, usually located in the big toe or the elbow. This impairs normal functioning and reduces the quality of life. What can be done to prevent gout attacks from occurring more frequently and causing less pain? Is a purely pharmacological treatment sufficient?

Gout represents a purine metabolism disorder whose clinical trajectory is profoundly influenced by dietary patterns. The underlying pathogenesis stems from impaired uric acid excretion—a terminal byproduct of purine catabolism, which constitutes a fundamental component of nucleic acids ubiquitous across all eukaryotic cells. Given the pervasive presence of purines in both plant- and animal-derived foods, excessive intake may lead to serum urate accumulation. Under physiological conditions, the body efficiently clears this surplus; however, genetic predispositions, environmental exposures, or behavioral factors can compromise renal elimination mechanisms, resulting in hyperuricemia—a chronic elevation of serum uric acid levels. Initially asymptomatic, this condition predisposes to monosodium urate crystal deposition within synovial spaces, ultimately triggering an autoinflammatory response that drives cartilage degradation and systemic inflammatory manifestations. Epidemiologically, gout correlates strongly with metabolic syndrome—characterized by visceral obesity, insulin resistance, and hypertension—disorders intricately linked to prolonged consumption of energy-dense, low-fiber diets with high glycemic loads. Primary triggers for acute gout flares include chronic alcohol consumption (particularly purine-rich beer), oxidative stress, inadequate hydration, and sedentary behavior. Clinically, attacks typically arise abruptly, often nocturnally, with a predilection for the first metatarsophalangeal joint (podagra), presenting with swelling, violaceous erythema, excruciating pain that precludes weight-bearing, and systemic symptoms such as fever or malaise. Episode duration ranges from several days to weeks, followed by spontaneous resolution; however, without lifestyle modifications and prophylactic pharmacotherapy, recurrences are inevitable, with intercritical periods progressively shortening over time.
