Nutritional deficiencies – foundational concepts, causative factors, and pathophysiological pathways
While conventional wisdom often attributes nutritional deficiencies solely to inadequate dietary patterns, the etiologic landscape is substantially more complex. Numerous pathological conditions—including but not limited to celiac disease, parasitic infestations, cystic fibrosis, chronic intestinal disorders, and eating pathologies—may impair the absorption of essential micronutrients and macronutrients despite an ostensibly balanced diet. An additional, clinically significant contributing factor is polypharmacy, which disproportionately affects geriatric populations. Elderly individuals frequently undergo multi-drug regimens, thereby elevating the likelihood of pharmacodynamic interactions that compromise nutrient bioavailability. For instance, bile acid sequestrants (e.g., colestipol) have been documented to reduce the absorption of folic acid, vitamin A, cobalamin, vitamin D, vitamin K, calcium, and iron (Skrypnik et al., 2013). Further predisposing factors encompass the chronic misuse of psychoactive substances (including alcohol and tobacco), sustained oxidative and psychological stress, as well as physiological states of elevated metabolic demand such as pregnancy and lactation. Crucially, the absence of overt clinical manifestations does not preclude the presence of subclinical deficiencies, as the human body employs compensatory mechanisms that may obscure nutritional gaps for prolonged periods. A case in point is calcium homeostasis: when dietary intake is insufficient, skeletal reserves are progressively depleted to maintain serum levels, a process that remains asymptomatic initially but ultimately predisposes individuals to osteoporosis and fragility fractures over time.
