SIBO syndrome, characterized by bacterial overgrowth in the small intestine, affects up to 22% of the adult population. Conventional treatment is based on targeted antibiotics aimed at reducing the number of bacteria colonizing the small intestines. However, this treatment should be supported by an appropriate diet, preferably FODMAP, SCD, GAPS or Dr. Siebecker's protocol. Small Intestinal Bacterial Overgrowth Syndrome (SIBO) represents a pathological condition characterized by the abnormal proliferation of bacterial strains within the small intestine—either through the colonization of microorganisms that are not typically present under physiological conditions or through the excessive multiplication of bacteria that normally reside there in only trace quantities. This dysregulated microbial growth gives rise to a constellation of distressing gastrointestinal symptoms in affected individuals, including recurrent abdominal pain, alternating episodes of constipation and diarrhea, troublesome gastroesophageal reflux, excessive flatulence, and abdominal distension. Beyond the digestive tract, SIBO may also manifest with extrasintestinal symptoms, such as depressive episodes, chronic fatigue syndrome, inflammatory joint discomfort, and various dermatological abnormalities.
The etiology of SIBO is multifactorial and intricate, with common contributing factors encompassing reduced gastric acid secretion (achlorhydria), pancreatic enzyme deficiencies, immunological dysfunctions, and structural anomalies within the gastrointestinal tract—such as strictures, neoplastic lesions of the small intestine, or the presence of diverticula. Furthermore, SIBO frequently coexists with other medical conditions, including celiac disease, Crohn’s disease, chronic liver disorders, and diabetic neuropathy.
The management of SIBO presents a clinical challenge due to the complexity of its underlying mechanisms and the propensity for recurrence. Conventional therapeutic approaches primarily rely on targeted antibiotic regimens, commonly employing agents such as metronidazole, doxycycline, and rifaximin, which have demonstrated efficacy in reducing the excessive bacterial load. However, an equally critical and complementary component of treatment involves the implementation of a carefully tailored dietary strategy, the primary objectives of which are to restore microbial equilibrium within the small intestine and alleviate the characteristic clinical manifestations associated with SIBO.
